Sunshine Foundation Rebate Submission Form

The Sunshine Foundation has partnered with Connected Home Direct to provide supporters up to $300 back when they sign up for T.V., internet, and phone. A donation to the Sunshine Foundation will be made when a homeowner signs up for new cable services through Connected Home Direct. 

 - Rebates are for customers signing up for NEW service. Customers who already have service are NOT eligible.
 - The request form below must be submitted before install is scheduled.

To receive your rebate and schedule your install:

2. All rebates for which you are eligible will be e-mailed to you.
3. A customer service representative will contact you within 24 hours to schedule your install.
